Let's not forget the sheer range of subgenres. Mprenge in a dark, dystopian sci-fi story is going to explore very different themes than mprenge in a fluffy coffee shop AU. In the dystopia, it might be about oppression, control of reproduction, and body horror—themes deeply entangled with patriarchal control and gender policing.
In the coffee shop AU, it's about cute misunderstandings and supportive friends. The former might have a lot to say about gender as a tool of power. The latter might not mention gender at all, treating the pregnancy as an adorable surprise that everyone accepts without question. So the trope's capacity for exploration is vast, but its execution is entirely dependent on the genre conventions surrounding it.

Honestly, I dip into mprenge occasionally when I want something that feels high-stakes and intimate. The gender politics usually aren't why I'm there. I'm there for the palpable sense of vulnerability and the intense protective instincts it brings out in the other lead. It's a shortcut to a very specific emotional atmosphere.
Whether it explores identity depends on the author's skill and intent. Some use it as a metaphor for bodily autonomy or queer parenthood. Others use it as a simple mechanism to get Character A to need Character B desperately. Both are valid storytelling goals. The exploration happens if the author wants it to; otherwise, it's just set dressing for the relationship drama we all signed up for.

The fanfiction side of this is huge, and it often treats mpreg as a plot device for extreme hurt/comfort or established relationship development. The gender angle is usually not the focus; the focus is on the character drama. How does this stoic character react when his logical worldview is upended by biology? How does this careless character step up when his partner needs him?
It's a test of the relationship under bizarre stress. Any examination of gender roles is incidental, emerging from how the characters already functioned in their dynamic. If one was already more domestically inclined, he might naturally take on more caregiving. The story reinforces their established personalities, not an external ideology about men and women. It's character-driven, not issue-driven.
