Like in 'The City We Became', it's not a person but the actual city's underworld that functions like a mafia, and the big reveal is who's really pulling the strings from the shadows isn't a don but a parasitic entity feeding on the chaos. That flipped the whole 'crime family' thing on its head for me.
Another twist that stuck was in a lesser-known noir where the feared boss spends the entire book as this off-page menace, only for the protagonist to finally corner him and discover he's a frail, dying old man whose reputation is entirely manufactured by his underlings to keep the operation running smooth. The power was never real, just a story everyone agreed to believe. That felt more unsettling than any bloodbath finale because it questioned the whole mythology of control these books build up. The twist wasn't about who the boss was, but that the boss, as a concept, was the real villain everyone served.
The double-agent twist is classic, but what really gets me is when the boss's vulnerability becomes the weapon. I read one where the boss's obsessive protection of his 'ordinary' daughter was the central mystery—turns out she wasn't his daughter at all, but the heir to a rival syndicate he kidnapped as a child and raised, and she'd known the whole time, waiting to dismantle his empire from within. The twist wasn't a sudden betrayal; it was the slow horror of realizing the entire relationship, the core of his humanity in the story, was her long con.
