Alina had just returned to her room at the Sterling Atheria after the dinner with Arthur. She was about to change and shower when her phone rang. The screen showed Dr. Philip Evert.

Her stomach tightened.

"Alina," he said immediately, urgency in his voice. "There's been an emergency at the Vaughn estate. Arthur, he suffered a heart attack, but it's more complicated. Initial scans indicate a critical aortic dissection. He's unstable and deteriorating rapidly."

Alina froze, her mind snapping back to the dinner she had just shared with him. What happened?  She pushed aside the shock. There was no time for questions or disbelief.

“He’s ready for surgery. The team stabilized him enough for imaging, but the dissection is severe. You’re the only surgeon with the experience to handle this. If we don’t operate immediately, he won’t survive.”

Alina’s pulse quickened. Thoughts of the Vaughn estate, Sebastian, and tangled family issues flashed through her mind, but she pushed them aside. Right now, it was Arthur’s life that mattered.

“Understood. Assemble the surgical team and prepare the OR for an aortic arch repair. This will be high risk. I’ll be there as soon as possible.” Her voice stayed steady, even as her heart raced.

“Everything will be ready,” Dr. Evert replied. “Travel safely.”

She ended the call and moved quickly. Within minutes, she packed necessary tools, went over the scans in her mind, and arranged for a ride. Every detail mattered.

By the time Atheria Medical Hospital appeared, lights flashing against the night sky, she had mapped the procedure in her head, anticipating every complication. Stepping out of the cab, she moved with precision. This was more than surgery, it was a test of skill, focus, and resolve. Alina Hart would not fail.

Inside, Dr. Evert met her at the entrance, tension in his expression. “Dr. Hart. I’m glad you’re here. Time is critical.”

Alina nodded, "What is his current condition?"

“He’s stable enough for surgery,” Dr. Evert said, voice steady, “but the dissection is severe, especially the aortic arch. Without immediate action, he won’t survive.”

He led her into the conference room, where the surgical team was already in place. Large screens displayed Arthur’s scans, vital signs, and notes for the procedure.

“Dr. Hart,” Dr. Evert said, gesturing toward the team, “this is your team. Follow her instructions exactly. I’ll assist only if needed.”

Alina moved with calm precision, briefing the team on each step and highlighting the critical points. Every word was measured, a quiet but clear display of authority.

One face stood out: Dr. Lena Moreau, whom she had met briefly at the convention. Lena’s sharp gaze seemed to question whether Alina truly deserved her reputation. Alina noticed, but kept her focus.

Adam stepped forward. “I’ll coordinate the monitors and imaging feeds. You have my full support, Alina.” She nodded briefly, reassured by his quiet presence.

Time was critical. Every second counted. A life depended on her focus.

Just then, the Vaughn family arrived. Emilia stepped forward, pale and tense. "Dr. ... Alina? You? If anything happens to my husband, you’ll be responsible. Are you sure you should be doing this?”

Alina met her gaze evenly, calm and unwavering. "If you want your husband to live, you'll let me do my work. If not... I can step aside, and you'll need to find another surgeon who can save him."

Emilia's sharp eyes lingered on her, arms crossed, a mix of fear and mistrust in her tone. "I... I just need to know this is the right choice."

“I’ve handled complex procedures like this before,” Alina said firmly. "I know what's at risk. Your husband's life comes first; that's my only concern."

Before Emilia could respond, Arthur's brothers arrived: Armando and Antonio, along with their wives, Rebecca and Martina. Whispers and tense nods filled the space.

“Is he stable?” Armando asked, brow furrowed.

“Stable enough for surgery, but critical,” Alina said, meeting each face briefly before returning to the screens.

Antonio's look hardened. "And you're the one performing this?"

“Yes,” Dr. Evert said. “Dr. Hart is the most qualified for this procedure. I’ve reviewed her experience with even more complex cases. There’s no one better.”

Alina’s voice stayed calm. “I am performing the surgery. The OR is ready. The team is briefed. Every minute counts. Every action must be precise.”

Armando and Antonio exchanged uneasy glances. The authority in both Alina's and Dr. Evert's voices left no room for argument.

Sebastian hovered nearby, silent, tense. He noticed her calm, steady authority, contrasting sharply with the storm inside him. Guilt, unresolved feelings, and his secret pressed on him. Watching her, he understood why she had become the surgeon she was. Vaughn family grudges and past betrayals didn’t matter here.

Rebecca and Martina exchanged uneasy looks. Emilia’s concern lingered, but she stepped back, letting the surgeon lead.

Dr. Evert gestured to the team. “All right. Everyone knows their roles. Follow her instructions. Time is critical.”

Alina returned to the displays, reviewing Arthur’s scans. Every measurement, anomaly, and possible complication ran through her mind. Personal stakes, family tension, memories from dinner, and the subtle tension with Sebastian, all were set aside. Life hung in the balance.

Sebastian’s eyes followed her, a mix of admiration and doubt. He clenched his fists, silently judging his past choices.

“Alina,” he said quietly, “I… I have every confidence in your skill. But can you save him?”

Alina met his gaze evenly. “I’ve saved him before. I know what to do. Trust the process and the team.”

He nodded, swallowing hard, the weight of his secret pressing on him. “I do. Just… bring him back.”

The room fell silent except for the hum of monitors and shuffling team members. Alina exhaled, calm and resolute. “It’s time. Let’s begin.”

The OR doors slid shut, cutting off the anxious murmurs of the Vaughn family. The smell of antiseptic, low hum of monitors, and Arthur’s flickering vitals filled the room.

She studied him for a brief moment, each breath a reminder of the risks. Every second, every decision could mean life or death. Yet Alina felt no hesitation, only the quiet certainty of her skill and focus.

Outside, the city slept, unaware. Inside, a legacy, a family’s history, and a man’s life hung in the balance.

Gloved hands ready, instruments in place, she exhaled. The first incision would decide everything.

Dr. Alina Hart was ready.
