It's tough. A lot of plots just mash the worlds together. For an actual twist, I think you need a story that uses the crossover to ask a question the original TMNT lore doesn't. I read one crossing the 2003 Turtles with 'Fullmetal Alchemist' that did this. The twist was that alchemy required Equivalent Exchange, but the Turtles' mutation was this unbalanced, chaotic event with no clear cost. The entire conflict revolved around that philosophical clash, not just fighting Homunculi together. That kind of conceptual friction is a sure sign of an original plot. I'd search for crossovers with series that have a strong, opposing internal logic to the TMNT universe.

The twist was that their 'master' Splinter was a missing-nin from the Village, tying their origin directly into Naruto's hidden history. It made both worlds feel intertwined, not just slapped together. You have to look for summaries that hint at a merged history or a fundamental change to one world's rules because of the other's presence. Those almost always promise a more thought-out twist than 'characters meet and fight a bad guy'.

Honestly, your best bet might be to look outside the usual superhero or action genres. Crossovers with slice-of-life or horror can force incredibly original twists. I got hooked on a 'TMNT/Animal Crossing' fic where the twist was that the entire Turtle family got isekai'd to a deserted island. The plot was them using their skills not for combat, but for community building—Donnie engineering the infrastructure, Raph farming and fishing, Mikey designing, Leo managing resources.
