I think a lot about the role of shame. The character often knows, on some level, that they're losing it, and they hide it. They perform normality while screaming inside. This double life—the outward mask and the inward chaos—is exhausting and accelerates the decay. The depiction includes these moments of lucid horror where they see themselves from the outside and are terrified by what they see. That metacognitive glimmer, before it's swallowed again, is devastating.
Isn't it fascinating how the setting often becomes a direct reflection of the protagonist's mind? A house isn't just creepy; its layout might start to warp, rooms appearing and disappearing, echoing the character's dissolving grasp on reality. The line between internal turmoil and external threat completely vanishes. You end up feeling a profound sense of isolation, trapped within a collapsing psyche that has reshaped the entire world. The horror isn't in a monster, but in the landscape of a mind turning against itself.
A lot of stories use art or creative output as a meter. The character might be a writer, painter, or musician. Their work becomes progressively more disturbed, abstract, or violent, providing an external record of the internal collapse. It's a way to show the decay to the reader (and sometimes to other characters) in a tangible form, while the character themselves might believe they are creating their masterpiece.
The prose style often morphs. It might start clean and descriptive, then become baroque and dense with metaphor as the character's thoughts spiral. Or it can become stark and minimalist, reflecting emotional numbness. The writing itself performs the mental state. You don't just read the character's thoughts; you read them in the syntax, the vocabulary, the paragraph structure. The form is the content.
The best ones show the character fighting it, which is the real tragedy. They use lists, rituals, rational deductions to try and rebuild the walls. Watching those coping mechanisms fail one by one is more heartbreaking than any sudden descent into madness. You see the intelligence and willpower being spent on a lost cause, which adds a layer of profound respect and pity for the character. The decay isn't passive; it's a battle they are destined to lose.

WARNING: GRAMMATICAL ERRORS MAYBE BE FOUND THERE AS ENGLISH ISN'T MY FIRST LANGUAGE. IT'S A DARK BOOK AND MALE LEAD MIGHT COME OUT A LOT CREEPIER SO DEAL WITH IT.
I'm drawn to stories where the trauma response is not anxiety, but profound numbness. The character feels nothing—no fear, no joy, just a hollow static. The horror here is existential and quiet. They might walk into dangerous situations without a flicker of emotion because they simply don't care anymore.
This creates a uniquely detached protagonist. The thriller elements—chases, threats, discoveries—happen to someone who observes them from behind a thick pane of glass. The tension comes from waiting for the feeling to return, knowing that when it does, it will be a tsunami of repressed emotion that could destroy them or finally set them free. It's a slow-burn, internal kind of suspense.
Okay but seriously, the audiobook narrations for some of these? A good narrator can convey that dissociated, fractured mental state just through tone, pace, and breath. It's a whole other layer of immersion.
