I see it as a conflict between illusion and disillusionment. The first part of the novel sets up illusions: Ibarra's illusion of a happy homecoming, his and María Clara's illusion of a future together, the illusion of a just social order. The rest of the book is the systematic, brutal dismantling of every single one of those illusions. The main conflict is the process of awakening to a terrible reality. The pain of the story comes from watching beloved characters lose their innocence and hope, one shattered illusion at a time.
