Yes, Maomao and Jinshi do ultimately end up together in The Apothecary Diaries (Kusuriya no Hitorigoto), though their relationship unfolds gradually over the course of the light novels and manga. The story is known for its slow-burn romance, focusing more on political intrigue and Maomao's investigative brilliance before romance takes center stage.
As the narrative progresses, Jinshi—a high-ranking and enigmatic imperial official—becomes increasingly drawn to Maomao’s sharp intellect and unique personality. While Maomao initially remains emotionally detached, her feelings for Jinshi develop subtly and realistically over time.
By the latest volumes of the light novel (which go beyond the currently available anime and manga chapters), the two characters reach a clear mutual understanding of their feelings. Although not formally married within the published story so far, they are effectively committed to each other, and future developments strongly imply an engagement or eventual union.
This outcome aligns with insights from official light novel translations and author interviews, which suggest that Maomao and Jinshi's relationship is central to the long-term arc of the series.
Why This Matters for Fans
The romantic development between Maomao and Jinshi is not just a subplot—it’s intricately tied to the character growth and thematic depth of The Apothecary Diaries. Fans who enjoy intelligent, slow-paced romantic arcs with political and cultural depth will find their evolving relationship especially rewarding.

I am a big fan of the sick man's little wife 's manga, and I think MaoMao and Jinshi or the story that they present is fascinating. In the comic it is hard to find a 'I am in love with you' moment, but also their relationship is very interesting.
At times however MaoMao 's rigid expression melts a little, and it is obvious that she cares for Jinshi. She spares no effort in trying to keep him safe. It is not the trite kind of romantic 'love' that most girls wear on aprons, but rather a nuanced bond filled with courtesy and affection.

Maomao's age is as enigmatic as her persona, which appeals to me as someone who enjoys the complex political maneuvering in ACGN. Based on her medical knowledge and experiences, she appears to be in her mid-twenties, though this is never clearly stated.
