I think the tension is most effective when the secret is, in some way, justified. Not morally right, but understandable. The protagonist is hiding because they're terrified, or to protect someone, or because of past trauma. If the secret is just selfish or stupid, the protagonist becomes unsympathetic. But if we, the readers, get why they're lying even as we hate the lie, that's the magic. We're torn, just like they are.

Honestly, sometimes I think it's overused. Not every romantic suspense needs a Big Secret. Can't tension just come from, you know, the actual suspense plot? I prefer when the tension comes from them working as a team against an external force, with all their cards on the table. The 'secret' trope can feel like a cheap way to manufacture conflict if it's not integral to the main plot.
