Isabella stared at the gold barrel of the pen.

It was just a piece of metal and ink. But in Diego's hand, it was a loaded gun pointed directly at her brother's head.

The massive executive suite was suffocatingly quiet. The only sound was her own ragged breathing and the faint, rhythmic tick of Diego's watch.

He didn't rush her. He didn't speak. He just waited, motionless, letting the silence crush the last of her resistance.

She reached out.

Her fingers were trembling. She hated herself for it, but she couldn't stop the shake.

She took the pen from him. It was heavy. Much heavier than the cheap plastic ones she used to chart patient vitals in the ER. It felt cold against her skin.

Isabella looked down at the cream colored paper.

Marriage Certificate.

The printed words blurred for a second. She blinked hard, forcing her vision to clear. She thought of her brother. She thought of him bleeding out on that warehouse floor weeks ago, of the supplies she had stolen to keep his heart beating.

This is the price, she told herself.

She pressed the gold nib to the paper.

The loud, crisp scratch of the ink echoed in the dead silence of the room. It sounded like a match striking in the dark.

Isabella.

She signed her first name. Her handwriting, usually a rapid, illegible medical scrawl, was slow and deliberate.

Diego watched her. He didn't smile in triumph. His dark eyes simply tracked the movement of her hand, absorbing her surrender with cold, corporate satisfaction.

Cruz.

She finished the signature.

With that single stroke of ink, her career, her freedom, and her life as she knew it were gone. She belonged to the Dallas underworld now.

Isabella lifted the pen off the paper.

Diego smoothly pulled the signed marriage certificate across the desk. He slipped it into the manila folder and closed it.

He looked past Isabella toward the heavy oak doors. "Javier."

The doors clicked open. The enforcer with the neck tattoo stepped inside. He didn't speak. He just walked up behind Isabella's chair and waited.

Isabella stood up. She turned and walked out of the freezing office, her hospital clogs silent on the thick carpet. Javier stayed exactly one step behind her.

The VIP elevator dropped them fast, bypassing the lobby and opening directly into the private, underground parking garage. The brutal Dallas heat hit her, heavy with the smell of exhaust and hot concrete. It clung to her thin scrubs, a violent contrast to the sterile penthouse.

Three massive, black Cadillac Escalades idled in a tight convoy.

Javier gripped her arm—not enough to bruise, but hard enough to prove he could—and guided her to the middle SUV. He pulled open the heavy, armored door.

Isabella climbed inside. Javier slid in right next to her, blocking her exit.

The door slammed shut. It locked with the heavy, mechanical thud of a bank vault sealing.

The interior was pitch black. The windows were tinted so heavily the blazing Texas sun couldn't penetrate the glass. The air conditioning blasted freezing air against her skin. The tight space smelled of expensive leather and the sharp, metallic scent of gun oil.

Isabella pressed her back against the seat, pulling her knees close together. She stared at the back of the driver's head. Nobody spoke. The only sound was the low, powerful hum of the eight cylinder engine as the convoy rolled out of the garage.

She watched the blurred city blocks pass through the dark glass. The driver bypassed the exit for the medical district. They weren't heading toward Trinity Memorial.

The SUV accelerated, merging onto the interstate and speeding away from the city center.

Isabella gripped the edge of the leather seat, her knuckles turning white. "Where are you taking me?"

Javier didn't turn his head. He kept his eyes locked on the road ahead, his hands resting casually near the bulge of his shoulder holster.

The silence in the vehicle remained absolute.

The Escalade slammed to a violent halt.

Before Isabella could catch her breath, Javier shoved the heavy armored door open.

"¡Ahhh! ¡Jefe, me muero!"

The raw, guttural scream shattered the silence.

Javier grabbed her arm and pulled her out. Isabella's hospital clogs hit cold, slick concrete. The damp, metallic stench of hot blood and spent gunpowder immediately coated the back of her throat, making her gag.

Three men lay on the floor among overturned wooden crates. One was thrashing, clutching a ruined thigh.

But Isabella's eyes locked onto the center of the warehouse.

Diego was kneeling in a pooling slick of dark red. The crisp white shirt from the penthouse was ruined, the sleeves rolled to the elbows and soaked through. His heavy Patek Philippe watch was smeared with thick, sticky blood. He was pressing both hands against the neck of a thrashing sicario.

"¡Quédate conmigo, hermano! ¡Mírame!" Diego roared. The veins in his neck bulged as he fought to hold the dying man down.

Isabella's stomach dropped. Her skin went ice cold.

The memory of the ink drying on that cream colored paper flashed in her mind. The reality hit her with a physical punch. She hadn't married a strict, untouchable businessman. She had married a butcher.

The man under Diego's hands choked. Bright red blood spurted up between Diego's fingers in heavy, rhythmic pulses, matching a frantic, dying heartbeat.

Isabella didn't think. Her feet moved.

She ripped her arm out of Javier's grip and sprinted across the concrete.

"You are killing him!" Isabella yelled. She dropped heavily onto her knees right into the puddle of blood.

She slammed her hands against Diego's forearms, shoving his heavy, blood soaked hands out of the way. "Move!"

"He's bleeding out!" Diego snarled. His eyes were wide, frantic, and murderous as she pushed him back.

"Because you're crushing his trachea!" Isabella shouted right back at him. She plunged her bare fingers straight into the torn, hot flesh of the man's neck. "You're suffocating him!"

She searched entirely by touch through the slippery muscle until she felt the severed vessel. She pinched her fingers down hard. The rhythmic spurting slowed to a weak seep.

Diego didn't just step back. He surged to his feet, a coiled spring of explosive, feral rage.

"¡Malditos sean!" he roared.

He kicked a heavy wooden crate, sending it splintering across the concrete floor. His hands dripped dark blood as he paced the center of the room like a caged animal.

"Lock down the ports!" Diego shouted at the armed men standing frozen in the shadows, his voice echoing off the high metal ceiling. "Find out who tipped them off! Nobody leaves this city!"

Isabella ignored the warlord pacing behind her. She kept her grip locked on the dying man's neck, her eyes fixed on the wound. The concrete was cold and gritty under her knees, and somewhere behind her a radio crackled with a soldier's tense, clipped voice, relaying the count of the dead.

"I need a clamp!" she demanded, glaring up at the men standing around. "Superglue, fishing line, a first aid kit—anything! Bring it now!"

Isabella didn't answer his question right away. She reached into the heavy red toolbox, grabbed a roll of black electrical tape, and quickly bound the bloody piece of fabric tightly over the sabotaged artery to hold the pressure."He's stable to move," Isabella said, wiping her bloody hands on her scrub pants. She looked up at the soldiers holding the flashlights. "Get him to a real hospital before he loses the leg."She stood up. Her knees ached against the hard concrete. She turned to Diego. He was still standing at the edge of the light, waiting for her."I need to speak with you," Isabella said. Her voice was flat.Diego's dark eyes narrowed slightly. He gestured to the open warehouse with a bloody hand. "Habla.""Not here," Isabella said, her tone leaving zero room for negotiation. "Alone."A muscle feathered in Diego's jaw. He wasn't used to being given orders by anyone, let alone a woman in bloody hospital clogs. But he gave a short, sharp nod.He turned and walked away from th

"I can pay for it."Isabella hated how small her voice sounded. She gripped the edge of the mahogany desk, her knuckles turning white. She refused to look away from him."I have money saved. And if you need off the books medical work, I am the best trauma surgeon in this city. I will work off the debt. You won't hear a word of complaint from me."Diego didn't laugh. He didn't even smile. Before you pass out on my floor."Isabella didn't want to submit, but her legs were shaking so badly she had no choice. She sank into the chair, the cold leather swallowing her."I don't need your money," Diego said, his voice dropping into a smooth, deadly rhythm. "And I don't need a basement doctor.
