Chapter 1

Amid the rowdy cheers of his crew, Leo got down on one knee and devoutly slipped the ring onto Nina’s finger.

The diamond ring looked flawless against Nina’s slender hand.

Once upon a time, I’d imagined Leo proposing to me countless times. Now, I stood on the sidelines like a stranger, watching him and Nina exchange happy smiles while my heart sank with sorrow and disappointment.

After the proposal, Leo came back to me and wrapped me in his arms to offer reassurance.

“Don’t read too much into it. Nina’s been diagnosed with advanced medullary cancer. Ever since she was a little girl, she has been dreaming about the day she would be proposed to. It’s her only wish. I’m just helping her make it happen.

“I need you to be the bigger person here.”

I murmured quietly, “Mm.”

Apparently, Nina’s dream mattered. Mine didn’t, though.

If that was how it was, I’d rather wake up from this dream.

Nina strutted over in her heels, the ring blinding under the streetlight.

She met my gaze tearfully.

“Ayla, I know you’re upset because I took away your proposal, but I swear I’m not making this up. I pray every day that this diagnosis is just a big mistake.”

Spotting her tears, Leo scowled and turned to me like I was a child throwing a fit. “Ayla, Nina wouldn’t joke about something like this.”

I kept my eyes on the ground. “Right.”

Refusing to drop the matter, Nina pulled a file from her bag and looked at Leo. “She won’t believe me! I brought my hospital records. Just look—”

Leo released me and pressed his hand against the folder. “There’s no need for that. I believe you.

“She’s just a junior doctor. She wouldn’t understand what’s in the file.”

I stood there in a daze, my fingers stiff with the November cold.

My mind went back to the night Leo and I first met.

He’d been shot in an attack by a rival outfit and rushed to Weill Cornell, where I removed the bullet and stitched him up.

At the time, I was a first-year emergency medicine resident. It was my first time removing a bullet on my own. My hands were shaking, but my technique never faltered. When I extracted the bullet with the forceps, he smiled.

“You’re pretty good at what you do, doc. You're going to be one of the best ER doctors out there someday.”

His words stuck with me. Even though several departments wanted me, I chose to pursue a career in emergency medicine.

“It’s getting late. It’s not safe for Nina to take a cab home on her own. I’ll take her back.”

Leo brushed a quick peck on my cheek before helping Nina into the car. He didn’t so much as look back to check on me.

Nina flashed a smug smile at me, her eyes gleaming with victorious disdain.

Saying nothing, I stared silently as Leo’s sports car disappeared around the street corner.

The November night air was bitter. As soon as Leo was gone, his men disappeared, leaving me standing alone in the middle of the square.

I took out my phone, opened my inbox, and located the email that had been sitting there for a while.

“International Medical Relief Mission—Emergency Deployment”

Due to a cholera outbreak in Central Adubrica, the mission was urgently seeking emergency physicians. After passing the qualification screening, I'd been added to the standby list.

I had planned to go, but the deployment date conflicted with my wedding. On top of that, Leo would never have allowed me to leave, so I put the opportunity aside.

Now, if Leo could propose to someone else, what was there that I couldn’t walk away from?

I opened the assignment notice. It was a five-year deployment, with my departure set for the following week.

I pressed the “Accept” button.

The screen refreshed with a message. “Your deployment has been confirmed.”

Seven days.

It was enough time to hand over my work, pack my bags, and disappear from Leo’s world.

Before long, the authorities were alerted to Leo's attempt to buy up the camp's medical supplies and interfere with the local healthcare system. He was deported. On the day of his deportation, he made one last stop at our camp. He was thinner, paler, and visibly exhausted. The commanding presence he once carried was gone. From where I stood, he looked almost like a defeated stray, lingering outside the camp and watching me from a distance. I was too busy seeing patients to notice, while Adrian stood right beside me, handing over instruments with perfect coordination. We worked so well together, oblivious to Leo’s presence. Nevertheless, Leo didn’t stay long. He was soon escorted to the airport. The days that followed were still hard, but I found joy in my routine. As I fought to snatch lives back from the jaws of death, a truth dawned on me. I hadn’t chosen emergency medicine because of a comment Leo had made. I did it because I valued human lives more than anything in the

Leo took a minute before something came to mind. “If it’s the contract you’re worried about, I can donate a sum of money, enough to build a high-spec hospital here. That way, you can leave with a clear conscience.” I felt nothing inside. He was nothing more than a familiar stranger to me. “Go ahead and donate.” My voice was flat. “This place can use a better medical facility.” He froze for a moment, a glimmer of hope flickering in his eyes. “But it doesn’t matter how many hospitals you build.” I looked Leo in the eye. “I’m still not going back with you.” His expression went stiff. “Ayla—” “Leo, you still don’t get it.” I cut him off. “I’m not staying here because of some contract. The people here need me.” His lips quivered, but no words escaped his throat. “What you need is someone who stays right where you left her, someone who gives in to you even after you give away her ring, her anniversary date, and her proposal.“I’m sorry, but I’m not that girl anymore.” Turn

As color drained from Nina’s face, she snatched the medical report from the nurse and tore it up. “What ridiculous lies! I have medullary cancer, and it’s advanced stage!” Eyes welling with tears, Nina turned to Leo and reached out to pull his sleeve. “Leo, they must have made a mistake. I can show you my old medical records.” Leo met Nina’s gaze. “Nina, we have some of the best medical care in the country, and you’re telling me that the doctors got your diagnosis wrong?” Nina, her face ashen, clutched Leo’s sleeve shakily. Leo pulled his sleeve from her grasp, his voice raspy. “Why did you lie to me?” Nina finally snapped. Her face twisted. “How else was I supposed to become the Donna of the Falcone family? Ayla’s just an ER doctor. What does she have that I don’t? She can’t even compare to me.” “She saved my life!” An elderly man stepped forward, supported by his family. “My heart stopped. Dr. Sinclair worked on me for more than an hour and brought me back.” “She

Third-Person POVLeo’s men exchanged uneasy glances, none breaking the silence until one spoke up in a hushed tone. “We only know Ayla boarded a flight, but we don’t know where she was heading.” Nina held onto Leo’s arm, her voice dripping with soft reassurance. “Keep it together, Leo. The hospital probably sent her out of town for an emergency conference. It’s not as if she hasn’t left without giving a heads-up before.” She sounded hurt. “That’s Ayla to you. She puts her job ahead of you.” Leo drew a deep breath. “You’re right…” Still, terror gripped him. Leo hasn’t received so much as a text from Ayla. In the past, no matter how mad she got during a fight, she had never cut him off without a word. “No, I need to find her.” Shaking off Nina’s arm, he grabbed his car keys and stepped out amid the uproar among the guests. Before Leo could make it out of the venue, Nina went limp and collapsed to the floor, her voice sobbing. “Leo! I can’t breathe… Everything hurts…” Leo

In the end, Christine bit her tongue, but the sympathy in her eyes said it all. It hit Leo that he had gone too far. Freaking out a little, Leo stepped forward to take my hand. “Ayla, I didn’t mean it that way…” I shied away from his grasp. “It’s fine.” I approached Nina’s feet and leaned down to examine her, desperate not to let anyone see the tears brimming in my eyes. Maybe this was for the best. I would let myself feel every bit of this pain before I walked away. After that, there would be nothing left to grieve. While I was bandaging Nina, Christine was called away to see another patient, and Leo stepped out of the ER to take a call. It was just Nina and me in the treatment room. Once certain that Leo was out of earshot, Nina lifted her foot without any trace of unease. “Honestly, it was just a tiny sprain, but Leo lost his mind over it anyway.” I packed up the equipment, not taking the bait. Nina chuckled. “You sure put up a lot just to marry Leo. You gave u

I squeezed a smile. “Be my guest.” Stepping out of the apartment, I noticed that Nina was already sitting in the front. Leo had brought Nina along. If he had already made up his mind, why bother asking for my permission? Once at the restaurant, Nina and Leo sat across from each other, while I sat next to Leo. When making the reservation, Leo had gone out of his way to order seafood as main courses, knowing they were my favorite. When the food was served, Nina threw her hand over her mouth, her face white. “What’s the matter?’ Leo tensed up. “I’m sorry… I can’t have seafood. Doctor’s orders.” She turned to Leo, her voice shaking. “I didn’t mean for this to happen… It just slipped my mind since I was so caught up in the moment.” I replied indifferently, “It’s okay. Save them for me.” Nina pulled a troubled look. “But even the smell makes me nauseous…” She clamped her mouth shut, looking as if she was about to throw up. Leo snapped his fingers at the server. “
