I actually have a contrarian take on this. A lot of the big-name bestsellers feel engineered for shock, and the suspense becomes predictable. My favorite kind of tension comes from slow-burn dread where the setting is the antagonist. Try 'Rebecca' by Daphne du Maurier. There's no physical threat for most of the book, just the oppressive weight of Manderley and the ghost of the first wife. The new Mrs. de Winter's anxiety becomes your own. It’s masterful.
Another one that doesn't get enough credit is 'I'm Thinking of Ending Things'. The suspense is existential and claustrophobic, built through disjointed dialogue and a creeping sense that reality itself is unraveling. It's less about what will happen and more about the terrifying realization that you can't trust the narrator's—or your own—perception.

If you want your pulse absolutely raced, skip the novels and dive into Stephen King's novella collection 'Full Dark, No Stars'. Especially the story '1922'. The suspense isn't in whether the crime will be discovered, but in watching the perpetrator's psyche disintegrate under the weight of guilt and paranoia. King builds a suffocating atmosphere of inevitable doom that's utterly relentless. It's a compact, brutal lesson in sustained tension.

Man, this question hits different for me because I've read so many thrillers that fizzle out in the final act. A story that genuinely held me in a vice grip recently was 'The Silent Patient' by Alex Michaelides. The structure is its secret weapon; you're constantly reassessing everything you think you know. It's not just about the 'who,' it's about the 'why,' and the pacing feels like a tightening noose. I made the mistake of starting it before bed and ended up finishing it at 3 AM, completely wrecked for work the next day. That's the sign of a plot that truly earns its suspense.
For a different flavor, I'd throw 'Gone Girl' into the ring. The suspense there isn't about a lurking monster, it's about the monster inside a seemingly normal marriage. The way Gillian Flynn manipulates perspective makes you complicit, and the infamous 'cool girl' monologue shifted my brain chemistry. The dread is psychological and deeply personal, which for me, sticks longer than any cheap jump scare a book can provide.

I get frustrated when a romance is just a series of longing looks and misunderstandings strung together for three hundred pages. For gripping plots, I immediately think of historical fiction that uses its setting as an engine. 'Outlander' is the obvious pick, but it’s obvious for a reason—the time travel element and Jacobite rising backdrop aren’t just scenery, they actively threaten and reshape the central relationship in brutal, irreversible ways. The stakes feel massive because they are.
Another layer down, I’d suggest 'The Seven Husbands of Evelyn Hugo'. Calling it just a romance feels thin; it’s a full biographical exposé where the love story is the explosive, hidden core that drives the entire narrative mystery. You’re turning pages to uncover the truth of it, not just to see if they get together. The plot is the excavation of that relationship, and the drama comes from the decades-spanning consequences of choices made in the spotlight’s glare. That structural trick makes it impossible to put down.
For a more intimate but equally plot-twisty option, 'The Last Letter from Your Lover' by Jojo Moyes weaves together two timelines where a journalist discovers old love letters, and the process of solving that historical romantic mystery directly impacts her present-day life. The ‘investigation’ provides a natural forward momentum that pure contemporary romance often lacks.
