Honestly? Often not enough of them, which drives me nuts. You get these plots where a character's major error causes a domino effect of misery for everyone else, but by the end they're kinda...forgiven? Redeemed by a last-minute sacrifice? I'm thinking of a certain fantasy series where a ruler's terrible misjudgment starts a war, and yeah, he's sad about it, but he still ends up in a position of respect. The consequences fall way heavier on the foot soldiers and side characters who trusted him.
I prefer stories where the architect of the mistake has to live in the ruined house they built. Not necessarily a bloody death, but a tangible, lasting downgrade to their life—loss of trust, respect, a forced exile from the world they knew. It's more satisfying and psychologically real than a tidy redemption arc that lets the audience off the hook.

Some mistake stories spread the blame around, but what's fascinating about something like 'The Secret History' is the way certain characters get crushed by the weight of their actions while others slip away. Bunny, obviously, pays the ultimate price, but it's Richard, our narrator, who I think faces the deepest, quietest consequences. He's the one who survives, who gets to tell the tale, but he's utterly hollowed out, forever an outsider looking in on the life he thought he wanted.
Henry seems to face a kind of consequence, but it's self-administered, almost like a logical conclusion to his philosophical experiment. The others—Francis, Charles, Camilla—they're damaged, sure, but they retreat into their wealth or their codependency. The real punishment lands on the character who had the most to gain and the least cushion to fall back on, which feels brutally true to life. Richard's mistake was wanting to belong to that world so badly he'd do anything, and the consequence is that he's permanently, profoundly alone in it.

A lot of the time it's the bystander or the conscience character who suffers most. The one who knew it was a bad idea but went along out of loyalty, or the one who tried to stop it and gets caught in the fallout anyway. Their consequence is the guilt and the 'I told you so' that nobody wants to hear. They have to live with the mistake and their own complicity, which is its own special kind of punishment.
