The healthy dynamics emerge from the latter approach, even if the packaging is all dragons and sword fights.
I find rivals-to-lovers often has the potential for very healthy dynamics. Why? Because they start as equals. They've seen each other at their most competitive, sharp-tongued, and strategic. The transition to lovers is built on a foundation of deep respect for each other's competence. The banter stays, but the intent shifts from harm to affection. That mutual admiration society feels incredibly healthy and adult.
A subtle point is the handling of past relationships or trauma. In unhealthy dynamics, a character's traumatic past is used to excuse their present abusive behavior. In healthier portrayals, the past is something they're actively working through, often with the support of their partner, who provides safety without trying to 'fix' them. The partner is a ally in healing, not a magical cure, and the narrative respects the complexity of that process.
