Inside that suffocating old warehouse, our emergency room was nothing but a dusty concrete floor, two hastily hung emergency lamps, and a rotting wooden table serving as an operating surface. The anesthesiologist, Dr. Valen, a middle-aged man with remarkably steady hands, crouched at the patient’s head. The portable monitor from the emergency kit beeped… beep… beep…, marking a heartbeat that was beginning to stabilize after the IV fluids.

“Blood pressure up to 90/60, saturation at 93 percent with nasal cannula,” Dr. Valen reported without looking up. “If you’re going to suture temporarily, now is the time. We don’t have long.”

I handed over sterile gauze, betadine, and the suture set. Ethan let out a slow breath, suppressing the nausea and weakness that still lingered from his illness, but his eyes were sharp.

“O’Connor 2-0. Tissue forceps. Sienna, retractor.”

“Ready.” My hands, oddly, weren’t trembling maybe because my brain was locked in a do-what-you-can-do-now mode.
