"We can't continue the surgery until someone pays the balance."

The words hit Serena Voss like a shock.

She stared at the doctor through the glass doors of the emergency room, her hands trembling as she clutched the strap of her worn-out handbag.

"I….I already paid yesterday," she whispered weakly.

The doctor sighed, exhaustion carved deep into the lines of his face. He was a good man –she could tell but he had to do his job.

"You paid part of it." He kept his voice low, as though it could soften what he was saying. "But your mother's condition suddenly worsened overnight. She needs another emergency procedure."

Serena's heart beat violently inside her chest.

"How much?"

He hesitated for a moment.

"$18,000."

Her knees nearly gave out.

She pressed one hand against the wall to hold herself, her fingers splaying flat against the cold plaster. Around her, the hospital continued its relentless rhythm as nurses walking briskly past, gurneys rolling, intercoms crackling with names and room numbers. The world kept moving as though hers hadn't just stopped completely.

Eighteen thousand dollars.

It might as well have been eighteen million.

She closed her eyes for one brief second, running through every number she had with her.

None of it came close.

"I don't have that kind of money," she admitted. Her voice broke but she kept her chin up. "Please... do the surgery first. I'll find a way to pay. I swear I will. I've never broken a promise to this hospital. Check my file. Please." She begged desperately.

The doctor's expression softened with something that looked uncomfortably like pity.

"Miss Voss–" He says softly. 

"She's all I have." Serena's voice dropped."Please. Just start the surgery. I will spend every hour working to pay what I owe. I swear on it in my life."

The doctor opened his mouth then closed it.

"I'm sorry." Before he could say another word, a shrill alarm split the air.

The sound was wrong in a way as Serena felt in her bones before she understood it.

Doctors and nurses burst through the double doors of the hallway, moving with urgency towards her mom's room as someone barked an instruction in medical shorthand she couldn't follow. Serena's breath caught in her throat as she whispered slowly. “Excuse me.” The doctor said as he ran with them.

"Mom..."

She ran with speed to her mom's room ward. She could see the doctors from the window as one doctor was performing chest compressions, leaning his full weight into each push.Another injecting something directly into the IV line. The heart monitor on the wall beeped a single unbroken tone.

Serena's forehead dropped against the glass.

"Come on," she breathed shaky. "Come on, Mom. Please. Please."

The compressions continued.

Then slowed.

Then stopped.

The doctor straightened slowly, stepping back from the bed.

He peeled one glove off then the other.

The room went silent in a way that needed no medical explanation. Serena already knew. She knew before the doctor came out and turned towards her with an expression that carried everything he was about to say before his lips moved at all.

"We're sorry."

"No." Serena cried slowly.

"We did everything we could–"

"No." The word came out again, harder this time, as though saying it firmly enough could reverse what had happened on the other side of that glass. "No, you didn't. You stopped. I saw you stop."

"Miss Voss–" The doctor tried to calm her.

"Check again." She grabbed the front of his coat with both hands, folding it under her fingers. "Please. Check again. She was stable two hours ago, you told me she was stable. She can't be – she promised me. She promised she'd get better. She promised she would see Mia graduate." Her voice cracked down as she broke down. "She promised."Tears streamed down her face in a way she couldn't control, couldn't slow, couldn't hide.

"Please," she begged, as her legs gave out under her.

She hit her knees on the hard corridor floor."I still need her." She said quietly.

No one answered as the nurses quietly lowered their heads. Some looked away. One young woman in blue scrubs pressed her lips together and stared at the floor, her eyes sad.

The silence that settled over the corridor suddenly felt hot as Serena felt choked. She held her chest in pain as the doctor and nurses rushed towards her in panic. Their shouts fading off.

Then darkness swallowed her whole.

When she opened her eyes again, slowly. A saline drip attached to the back of her left hand. She blinked slowly again taking in her environment.

She was in the hospital. As she remembered everything that had happened this morning. 

"So..."

Her voice came out cracked and rough."I'm still alive."

A single tear traced a slow path from the corner of her eye down into her hair.

"But Mom isn't."

She covered her face with both hands as a sob rose from somewhere deep in her chest and it wasn't a small, controlled sound, it shook her whole body, releasing everything that she had been holding back for months without realizing how much it had accumulated.

For twenty-five years, everything she had done had been in service of one purpose.

Her mother.

She had dropped out of college in her second year when the medical bills first began arriving with thick envelopes.

She had worked double shifts at Club Hollow for two years straight, coming home in the grey hours of early morning smelling like cigarette smoke, eating whatever was left in the refrigerator because she was too tired to cook.

She had skipped meals and worn the same three pairs of shoes in rotation because getting new ones was a luxury that existed somewhere beyond the edge of her current life.

She had borrowed money from people she knew she shouldn't borrow from, signed her name to things she understood were dangerous because the alternative was worse.

Every single sacrifice and still.

She had lost her.

"What was the point?" she whispered to the empty room.

The words came out one at a time, each one heavier than the last.

"I gave up every fucking thing." She yelled out as her shoulders shook so hard the hospital bed trembled with her. "And I still couldn't save you."

Her cries echoed off the walls of the empty room and came back to her unchanged, unanswered, absorbed by nothing.

Outside the window, rain had begun. It fell against the glass in long, uneven streaks, running sideways when the wind caught it. The sky looks like it was grieving with her.

She cried until she had nothing left.

And then she lay there in silence, staring at the ceiling, breathing heavily.

One Year Later

The photograph on her bedside table had collected a thin film of dust along the top of the frame.Serena noticed it as she picked up the frame carefully, the way she always did, and wiped the dust away with the edge of her sleeve.

Her mother looked back at her from inside the glass.

Young.

Healthy. 

Beautiful.

Her smile was wide and unguarded before the illness had made every expression cost something.

Serena traced the edge of the frame with the tip of her finger.

"I still miss you every day," she whispered softly.

The words were the same just as she has always said and she meant them just as much every time.

A knock at the bedroom door interrupted the silence.

She set the photograph back quickly, pressing the backs of her hands briefly, cleaning the tears against her cheeks.

"Come in."

The door creaked open several inches as Mia's face appeared in the gap.

"Sis..." She called out.

Serena distracted herself as she pretended to fold some clothes."What is it?"

Mia pushed the door open fully and came to sit beside her on the bed, which was how Serena knew it was something about money before she'd said a word.

"Our school excursion is tomorrow."

"I remember."

"I... I actually need to go." She looked down at her hands folded in her lap. "The teachers said some of the exam questions will come directly from the trip. It's not optional, really."

The room went quiet.

Mia hated asking for money. Serena had watched her sister develop a sense of understanding over the past year, she had watched her learn to go through things rather than add to the weight her sister carried. She recycled notebooks. She declined invitations to things that cost money. She pretended she didn't want things that other girls her age wanted without question and sometimes that broke Serena's heart more than anything else.

"I understand if we can't," Mia said softly, still looking at her hands. Serena glanced at her phone.

The banking app was already open from earlier this morning with the grimly practical hope that the number would somehow be different from what she remembered.

She looked at her sister. Mia was holding herself very still, bracing for disappointment, already preparing to say it was fine. Serena smiled.

"You'll go."

She transferred the money before Mia could argue.Mia's phone chimed.

She looked at the screen.

Then at Serena.

"Sis..." Her voice went small. "You didn't have to–"

Serena reached over and pinched her cheek the way she had since Mia was four years old.

"Go enjoy yourself."

Tears filled Mia's eyes immediately, the way they always did when she couldn't find the words for what she felt.

She threw both arms around Serena's neck and held on hard.

"Thank you!"

Then she was gone–door swinging, footsteps quick and light down the hallway, her voice already rising as she called one of her friends to tell them the news. Serena looked at her phone.

She felt on the mattress and drew a long, slow breath.

Tonight's shift would have to cover all the bills for the week until her next paycheck. She also has unsettled debt with Victor Crane.

The name surfaced in her mind the way it always did–uninvited, cold,dread in her stomach.

Victor Crane.

A deadly loan shark. The man who had given her the $50,000 when no one else would,without blinking and told her the repayment terms.She had already missed two payments and Victor Crane was not known for understanding. He was not known for patience and for giving second chances to people who couldn't keep their word.

A cold shiver moved slowly down her spine.

She opened her eyes and started getting ready for work.

"I just have to survive tonight," she murmured to the empty room.

She had no idea that before sunrise, everything was going to change.

That somewhere across this city, the most powerful man in New Orleans had already seen her face.

And couldn't stop thinking about her.
