Reina gadis 18 tahun, memutuskan pindah ke Pelombo, suatu desa terpencil di Gunungkidul bersama keluarganya. Disana Reina dan keluarga tinggal di rumah nenek mereka, Nenek Iroh. Namun warga desa tidak suka dengan keberadaan keluarga Reina, warga pun mengucilkannya. Mereka menganggap keluarga Reina dapat membawa malapetaka. Suatu hari Reina mengumpat dan meludahi sumur tua di rumah neneknya itu, dari sini mulai lah kejadian-kejadian mistis terjadi. Reina di hantui oleh mahkluk halus, Nenek Iroh jatuh sakit dan badannya seperti mayat hidup. Akhirnya Pak Arya (Papanya Reina) menemui Mbah Darmo, dukun yang terkenal sakti di desa itu untuk memusnahkan mahkluk halus yang mengganggu keluarganya. Sayangnya Mbah Darmo tewas, tak mampu mengalahkan mahkluk itu. Kemal (Adiknya Reina) juga hilang secara misterius. Membuat Bu Mirna (Mamanya Reina) hampir gila. Reina memutuskan mencari Kemal bersama Bagas, teman baru Reina di desa itu. Secara diam-diam mereka juga mencoba menguak misteri sumur tua itu. Mereka bertemu dengan Nenek Bejo, adik dari Nenek Iroh. Nenek bejo menceritakan semua kejadian di masa lalu yang penuh dendam. Sumur tua itu ternyata sarang iblis jahat yang mempunyai dendam di masa lalu. Nenek Bejo ingin menyerahkan nyawanya kepada iblis itu untuk di tukar dengan raga Kemal agar Kemal kembali kepada keluarganya. Hal itu Nenek Bejo lakukan untuk menebus kesalahannya di masa lalu kepada Nenek Iroh.
